North Bristol NHS Trust is the largest Trust in the South West region, and one of the largest trusts in the country.  Last year the Trust treated over 100,000 inpatients, over 300,000 outpatients, over 90,000 emergency department patients and helped deliver nearly 5,000 babies.  It employs more than 8,500 staff, has approximately 1300 inpatient beds and a revenue budget in excess of £400 million.

The Trust provides medical surgical, paediatric and maternity care for its local population of approximately half a million people in the North Bristol and South Gloucestershire area.  The Trust also provides sub regional, regional and supra regional services in the fields of neurosciences, orthopaedics, pathology, plastic surgery and burns, renal medicine and transplant and urology.

The health service in Bristol, North Somerset and South Gloucestershire is being remodelled to concentrate acute facilities and where appropriate to provide more care close to patient homes through a network of community hospitals and facilities.  The Trust currently delivers services from 2 acute hospital sites, Southmead and Frenchay.  A major investment has been agreed that will concentrate these services onto a new hospital on the Southmead site by 2013.

The Trust has University Teaching status and is associated with both the University of Bristol and the University of the West of England.

Haematology

There are 4 consultant Haematologists across the Trust with an active Day unit and Specialist Nurse, based at Southmead Hospital. There are plentiful opportunities for learning about general haematology and Haematological malignancies through out-patients, day care patient management and in the in- patient setting. There are 2 Specialist Registrars, 2 associate Specialists and an active MDT.

 

Respiratory
 
Respiratory medicine is based in the North Bristol Lung Centre at Southmead Hospital. The department has a large team of specialists including 6 consultants, 13 specialist nurses and physiotherapists, a team of junior medical staff and two specialty wards with integrated out-patient facilities. Acute Medicine
 
Acute Medicine is located at both Southmead and Frenchay Hospitals at North Bristol Trust.  Each site has a fully functioning MAU with admission/assessment functions, and a Seated Assessment Area.  There are dedicated consultant-led teams in Acute Medicine who work with the on-call team.  The MAU offers trainees ample opportunity when on-call to acquire the requisite experience and skills for CMT training.
 
Renal Medicine
 

There are 10 Nephrology consultants divided into two firms for the management of renal inpatients. Doctors working in the renal unit are well supported and the post provides excellent experience in all aspects of renal medicine. Doctors will leave with an awareness of the range of causes and presentations of acute kidney injury and experience of managing these acutely unwell patients. They will also have exposure to the investigation and management of other renal conditions such as nephrotic syndrome and vasculitis, and also have involvement in the management of newly transplanted patients. Considerable general medical training is also provided in the inpatient care of complex long term dialysis and renal transplant patients.

We have weekly multidisciplinary teaching ward rounds, weekly X-ray meetings and renal histology meetings and weekly in-house specialty renal tutorials as well as bimonthly renal academic meetings.

Care of the Elderly

Our department manages all aspects of the care of older people in hospital.  Given the ageing population, experience in this field is useful for all hospital medical careers, not just those interested in specialising in medicine for the elderly.  

There are currently five and a half (WTE) consultants, two and half (WTE) stroke physicians and five registrars, working across the two sites. Older people are assessed and managed in wards with a proactive approach to their treatment and rehabilitation.  Weekly multidisciplinary meetings are held on all elderly care wards, giving experience of the issues surrounding the inpatient rehabilitation and discharge planning in elderly patients. 

The department also offers several specialist services across the two sites, including the investigation and management of Parkinson’s Disease, falls, orthogeriatrics, elderly care surgical liaison, cerebrovascular disease, cardiovascular disease, respiratory medicine in the elderly and dementia. There is a strong emphasis on education, with links to the university and the opportunity to teach both on the wards and in workshops. 

Frenchay hospital is in the forefront of national stroke thrombolysis and offers excellent training in acute stroke management.

Each site has a lively post-graduate meeting within the department each week, at which articles and cases are discussed.  There is a weekly Grand Round on each site, and different sub-specialisms have arrangements with radiology for x-ray meetings, which provide educational opportunities.

Gastroenterology 

There are 4 Gastroenterology consultants who lead teams of specialist doctors, nurses and professionals allied to medicine. The inpatient work includes caring for patients with GI bleeding,  inflammatory bowel disease, nutritional problems and acute and chronic liver disease. The team works closely with the GI surgical unit and there are regular multidisciplinary histopathology, radiology and journal club meetings. The weekly outpatient clinic provides experience of the less acute aspects of Gastroenterology including chronic liver disease, functional bowel disease and less acute IBD. There is also a day hospital where procedures such as ascitic drains and infliximab infusions are regularly carried out.

Rheumatology

Rheumatology Post, Trym Ward, Southmead Hospital

This 4 month attachment offers great experience of the multidisciplinary team management of a wide range of rheumatic diseases. You will be based on Trym Ward in the Avon Orthopaedic Centre, where all rheumatology inpatients from Bristol and Weston-super-Mare are managed by the team which includes specialist physiotherapy, occupational therapy, nursing and psychology colleagues. As well as the ward work, there is a rheumatology out-patient clinic each week where, after a period of observation, you will have your own list of patients to see each week. There are opportunities to do audit. The post holder also participates in the acute medical on-call rota. Previous post holders have become confident in musculoskeletal examination and common joint injections and gained useful experience in the management of different types of pain.

Infectious Diseases 

The ID unit at Southmead Hospital consists of a 10 bedded ward ( all isolation cubicles) and a dedicated HIV/Immunology outpatient department (Brecon Unit). The Consultant led team ( Dr Glover, Gompels, Bovill and Johnston ) consists of 1 SpR ( Immunology/Infectious Diseases/GUM), a F2 or ST 1/2 doctor and a  F1 doctor. Southmead  Hospital  runs an   unselected medical take with triage of appropriate patients to the ID Unit. The main interest of the unit is HIV/AIDS  but it will also deal with fevers in the returning traveller, fevers of unknown origin as well as the more common infections such as pneumonias, meningitis, soft tissue infections and complex Tuberculosis. Dr Glover is on the general medicine on call rota and thus has responsibilities for outlying general medical patients on other named wards.

Diabetes & endocrinology for core medical trainees in North Bristol

North Bristol has a comprehensive diabetes & endocrinology service, with diabetes centres housing the multidisciplinary teams & specialty wards on each site & is an academic centre of excellence for diabetic medicine. The team includes 6 consultants (2 of whom are professors of diabetic medicine), diabetes nurses, specialist dieticians & podiatrists. Trainees will gain experience in the management of inpatients with diabetes, diabetic emergencies & diabetic foot problems with support from inpatient diabetes nurses & the diabetic foot care team. There is also a lot of inpatient & emergency endocrinology with services for pituitary surgery, neurosurgery & endocrine surgery in the Trust. They will attend one diabetic clinic per week & be encouraged to attend some of the wide range of other specialist diabetes & endocrine clinics.

Cardiology for core medical trainees in North Bristol

Cardiology services are provided at both the Frenchay and the Southmead sites.  There are 6 Consultant Cardiologists with various specialist interests spanning coronary intervention, electrophysiology and pacing, echocardiography, valvular heart disease and heart failure.  The cardiac catheter laboratories are based on the Frenchay site and coronary intervention including PCI and device implantation happen throughout the working week.  Both hospital sites have a CCU and cardiology ward providing care to acute admissions and to patients following elective procedures.  At the Southmead site, there are weekly specialist educational meetings as well as a weekly SHO heart failure clinic which is well supported by a Consultant. SHOs at the Frenchay site regularly attend general cardiology outpatients. They have opportunities to supervise exercise tolerance tests and to observe the range of cardiac procedures which take place on site. There are many opportunities to perform audit projects relevant to cardiology.

Neurology for core medical trainees in North Bristol

The Department of Neurology is based at Frenchay Hospital and benefits from excellent working ties with our colleagues in neurosurgery, neuroradiology, neurophysiology, neuropathology, neuropsychology and neuroscience research at Bristol University. We are an enthusiastic team of 11 consultants, 5 SpRs, 4 Core Medical or GP trainees and 1 F2. Our tertiary care territory extends from Taunton to Gloucester. All of our consultants have a sub-specialist interest.

We aim to provide excellent neurological experience and training for all our junior staff. Training opportunities include weekly formal neuroscience teaching, informal bed side teaching during ward rounds and radiology meetings. Study leave for appropriate courses is encouraged. For candidates taking PACES we are delighted to offer neurology specific PACES coaching from our enthusiastic team of registrars and consultants."
